Depuis son élection, Macron joue aussi avec impôts et taxes :

huit nouveaux dévoilé par ont été Ernst&Young, le chef de l'Etat a instauré au moins un nouvel impôt ou taxe par mois depuis son élection !

Pour ce faire le cabinet d'audit Ernst&Young d'audits'est appuyé sur la loi de finances 2018 et les lois de finances rectificatives pour 2017 pour arriver à cette conclusion.

Dans la liste des nouveaux impôts figure celui sur la fortune immobilière, qui vient remplacer l'impôt sur la fortune (ISF). On y retrouve également deux nouvelles "contributions exceptionnelles sur l'impôt sur les sociétés des très grandes entreprises". Ces dernières devraient financer l'annulation de la taxe de 3% sur les dividendes.

Petit doute:la taxe d’habitation serait-elle remplacée par un autre impôt??

En outre, cinq autres nouveaux prélèvements certains:

1° une taxe sur le tabac

2° une autre sur les plus-values de ventes de logements HLM,

3°un prélèvement "sur le développement des industries de fabrication de papier"

5°deux taxes "sur l'exploration d'hydrocarbures et de gîtes géothermiques".

Un gros doute demeure encore : la taxe d’habitation sera-t-elle remplacée par un autre impôt comme on l'a entendue sitôt démentie. Trop prématurée peut-être pour bafouer un idée de campagne.

Espérons que le président Macron restera moins créatif à ce jeu là que son prédécesseur qui avait mis en place pas moins de 28 nouvelles taxes et impôts entre 2012 et 2013 !

Une étude de l’Assurance maladie mesure l’impact des principales pathologies sur les dépenses de santé et projette leur évolution d’ici 2020.

L’Assurance maladie a donc présenté mercredi 31 mai sa cartographie des pathologies et des dépenses de santé en France en 2015. On passe au scanner les dépenses de santé des 57 millions de bénéficiaires du régime général.

Les soins de santé mentale pèsent toujours très lourds ; Une augmentation des dépenses liées aux cancer..7 à 8 millions d’assurés qui, a priori, ne dépensent rien…

Que signifie une telle "cartographie" ?

Pas de cartes à proprement parler dans ce bilan, mais de nombreux tableaux passant au crible les dépenses de santé des 57 millions de bénéficiaires du régime général.

"L’idée est de quantifier au mieux les pathologies ou les traitements les plus fréquents et les dépenses qui leur sont associées dans un contexte de tension persistante sur notre système de santé, explique Luc Barret, médecin conseil national. Ce ne sont pas des données médicales, mais on peut en tirer des informations médicales."

Pour la première fois depuis la création en 2012 de ce bilan annuel : une évolution des dépenses sur les quatre dernières années et des projections d’ici 2020.Et c'est surtout ce tout premier bilan qui intéresse le plus les gestionnaires!

Car ne perdons pas de vue que les enjeux de la santé publique et des coût qu'elle génèrent posent problème. Est-il besoin de rappeler le »trou » de la Sécurité sociale ,et surtout de la branche maladie ?

Ne nous voilons pas la face, cette cartographie n'est pas sans efficacité. Quels enseignements tirer de cette étude? De quelles maladies ces 57 millions de Français souffrent-ils le plus ? Comment se répartissent les 133,6 milliards d’euros dépensés en 2015 ? Quelles évolutions entre 2012 et 2015 ? Quelles perspectives ? Et surtout que dérembourser?! Qui faire payer?

On se répète mais avec un raisonnement à la clef.

20 millions de personnes soignées pour une pathologie au long cours ; Les Français vivent plus longtemps mais ils souffrent aussi plus souvent de maladies chroniques qui pèsent lourd dans les comptes de la Sécu.

Diabète, cancers, maladies respiratoires chroniques, maladies cardiovasculaires… Ces pathologies, nécessitant une longue prise en charge, concernent aujourd’hui 20 millions de personnes, soit 35% des assurés. Si on ajoute les maternités, les hospitalisations ponctuelles et des traitements anti-inflammatoire chroniques, ce sont 26 millions de personnes qui sont suivies régulièrement.

On chiffre :

10 milliards de dépenses supplémentaires entre 2012 et 2015

Pendant cette période, ce sont les dépenses d’hospitalisation qui ont le plus augmenté (+ 2, 1 milliards), suivies par les dépenses liées aux cancers (+ 1, 5 milliard) et aux maladies cardioneurovasculaires (+1,2 milliard)

19,3 milliards d’euros de remboursements pour la santé mentale

Avec 19,3 milliards d’euros de remboursements en 2015, les maladies psychiatriques arrivent en deuxième position dans le top trois des pathologies entraînant le plus de dépenses alors même que le nombre de personnes soignées a diminué de 102.000 entre 2012 et 2015.

Seules les hospitalisations entraînent plus de dépenses (30,7 milliards), tandis que les cancers viennent en troisième position (14,1 milliards.)

"Lors de notre première étude, ce poids déjà élevé des dépenses de santé mentale nous avait surpris, détaille Luc Barret. Il reflète notamment les importantes prescriptions d’antidépresseurs dans notre pays. Ces prescriptions sont moins élevées dans d’autres pays qui, eux, remboursent davantage les psychothérapies."

De ces données hommes-femmes, il ressort que les pathologies varient avec l’âge mais aussi en fonction du sexe. Ainsi les femmes sont globalement moins diabétiques et un peu moins touchées par les maladies respiratoires chroniques. Et même si elles sont en augmentation chez elles par rapport au passé, les maladies cardiovasculaires les épargnent encore un peu plus.

12.035 euros, le coût moyen du traitement d’un cancer du sein

En 2012, ces dépenses s’élevaient en moyenne à 11.288 euros. Cette augmentation est due à l’arrivée sur le marché de nouveaux traitements plus coûteux. En moyenne, le traitement d’un cancer, en 2015, était de 11.000 euros par an et par patient.


7 à 8 millions d’assurés ne se sont fait rembourser aucun soin en 2015

Ont-ils eu recours à des traitements non remboursés ? Négligent-ils leur santé ? Ou sont-ils tout simplement en 5% des hommes de plus de 75 ans ont une maladie cardioneurovasculaire...

Cette étude permet de mesurer l’impact des principales pathologies et de projeter l’évolution des dépenses pour les trois prochaines années.

Premier constat, en quatre ans, les dépenses de santé ont augmenté de 10,2 milliards d'euros pour atteindre 133,6 milliards en 2015, selon cette étude qui passe en revue 56 pathologies.

Plus d’un bénéficiaire sur deux a recouru à un soin courant, le reste de la population, soit 26 millions d’assurés, ayant été concerné par des soins liés à des pathologies chroniques, des traitements médicamenteux spécifiques au long cours ou la maternité .

Hospitalisations ponctuelles plus nombreuses

Les grandes tendances observées depuis 2012 se confirment avec "un recours important et croissant" aux hospitalisations ponctuelles sans lien possible avec une pathologie spécifique (+335.000 personnes sur quatre ans). La hausse du nombre de ces hospitalisations s’explique par le vieillissement de la population, en fait le premier poste de dépenses (30,7 milliards d’euros).

Autre phénomène confirmé par l’étude, "le poids de la santé mentale" (traitement des maladies psychiatriques ou consommation de psychotropes) avec des dépenses versées à plus de 7 millions de personnes atteignant 19,3 milliards d’euros (soit +2,3% en moyenne chaque année entre 2012 et 2015). "Grâce à ces données (…) nous sommes en train d’expérimenter des prises en charge psychothérapiques pour les épisodes dépressifs légers afin d’éviter le recours aux médicaments" explique l’Assurance maladie. La question est d’autant plus cruciale que le nombre de personnes atteintes de maladies psychiatriques devrait augmenter de 11% d’ici 2020 (soit 246.100 patients en plus)

Pour la première fois, l’Assurance maladie a présenté des projections à partir des prévisions démographiques de l’INSEE entre 2016 et 2020 et des évolutions épidémiologiques prévisibles. D’où il ressort que le nombre de malades chroniques ou nécessitant un traitement long (diabète, cancer) devrait augmenter fortement d’ici à 2020 – environ 580 000 personnes touchées de plus. En effectifs, les malades cardiovasculaires seraient toujours les plus nombreux en 2020 (5.114.000 contre 4.510.000 en 2015) et en pourcentage, ce sont les maladies inflammatoires ou rares ou VIH ou sida qui devraient le plus augmenter (+20%). Dans le cas du diabète, le nombre de personnes concernées devrait augmenter de 12%, soit 450.000 patients supplémentaires pour un total de 4 millions de diabétiques en 2020.

Dépenses de santé: l’Assurance maladie face aux défis des maladies chroniques

L’Assurance maladie estime que d’ici à 2020, 548.000 personnes de plus qu’en 2015 devraient être atteintes d’au moins une pathologie chronique ou être concernées par un traitement au long cours. Selon les estimations, le nombre de personnes traitées pour du diabète augmenterait de 12% entre 2015 et 2020. En 2020, plus de 4 millions de malades seraient touchés.

Les maladies cardio-neuro-vasculaires devraient elles aussi concerner 604.000 personnes de plus, mais pour les deux tiers cette augmentation s’explique par un vieillissement de la population.

L'Assurance-maladie donne aussi des projections d'évolution des principaux postes de dépenses par pathologie de 2015 à 2020. Elle exposera ses pistes d'économie début juillet.

Chercher des gisements de productivité.

Car à n'en pas douter, il faudra encore chercher des gisements de productivité. « La tension persiste sur le système de santé », a expliqué le professeur Luc Barret, médecin-conseil, lors de la présentation de ces chiffres. Les personnes touchées par les principales pathologies vont continuer à croître plus vite que la population et plus vite que le vieillissement

Le recours accru à des infirmiers en ville permet d'éviter des hospitalisations. C'est peut-être la preuve de l'efficacité du « virage ambulatoire », avance la CNAM. A l'inverse, les médicaments innovants peuvent faire déraper la facture à court terme.

Le recours aussi à l'augmentation de la CSG de 1,7% ne semble seulement qu'une petite entrée en matière ...

Et un système de santé à deux vitesses s'installe peu à peu.

Question : Pourquoi ne pas s'attaquer tout d'abord aux causes de l'augmentation des pathologies dans notre vie quotidienne ? Pollution, OGN , pesticides, stress ...

Le poids croissant du secteur privé confirme l’orientation vers une économie de marché rendant le maintien d’un environnement économique stable impératif notamment en ce qui concerne les prix...En fait la bourse chinoise ne reflète guère l'économie  «  réelle » du pays.

Pourquoi parler de « krach » bousier de Shanghai dans les médias occidentaux?

L’effondrement des marchés asiatiques des derniers jours d’août 2015 est qualifié par certains analystes de « krach » . C'est juste techniquement puisqu'il s'agit, effectivement, d'une baisse soudaine et précipitée des actions (plus de 20 % en quelques jours) touchant une ou plusieurs places financières. La Bourse de Shanghai a perdu plus de 40% depuis mi-juin.

Les soubresauts de la bourse de Shanghai sont impressionnants: une chute de 30% en un mois qui n'est pas sans rappeler les krachs de 1929 ou 2008. Pas moins de 3500 milliards de dollars se sont évaporés. Mais il faut néanmoins la mettre en perspective et se souvenir de la progression fulgurante des mois précédents. En effet, malgré cette correction, sur un an, l’indice de la bourse de Shanghai est quand même en progression de plus de 70% : il était à peine au-delà de 2000 points l'an dernier pour franchir la barre des 5000 points ce printemps. La hausse a donc été de 150% en moins d’un an, avant d’osciller entre 3373 et 4317 points en un mois. Rien d' inquiétant pour les économies occidentales!

Les autres places asiatiques se sont comportées à peu près normalement :Tokyo a rebondi par exemple de plus de 3% après six séances négatives alors que Hong Kong a terminé en baisse de 1,52. La répercussion le lundi sur les marchés européens et américains a été atténuée le lendemain. Le CAC 40 ayant notamment clôturé en hausse de 4,14 % !

Rien d' inquiétant finalement pour les économie occidentales! Les crises d’angoisse dans bien des médias ne sont hélas pas bien rationnelles. Elles apportent leur nouveau lot de mauvaises nouvelles, de peurs théâtrales censées provoquer un effet de catharsis !

Qui est réellement concerné? Faut-il craindre un effet domino sur l’économie réelle ?

Les petits porteurs...chinois principalement le sont.

Les investisseurs institutionnels à la Bourse chinoise ont vocation à conserver leurs portefeuilles et n'ont donc pratiquement rien perdu.

Par contre de nombreux ménages chinois après avoir investi dans l’immobilier, se sont tournés ensuite vers la Bourse. Des petits joueurs ont «  acheté les yeux fermés en levier [un mécanisme permettant de jouer plus gros que la somme misée] et se sont fait liquider en panique à la moindre correction », explique brutalement Nicolas Chéron, stratégiste chez CMC Markets France. Cette correction a eu lieu « après le plus que doublement de la valeur d’entreprises inscrites dans une conjoncture de moins en moins porteuse ». D’où les soubresauts de la bourse de Shanghai dernièrement puisqu'ils déterminent l’essentiel des mouvements de cours . Mais ces petits porteurs( 15% de la classe moyenne) ont payé un très lourd tribut...Et qu'en est-il des pays occidentaux ?

L’économie française reste somme toute est épargnée

« Il faut rester vigilant pour les entreprises qui sont très exposées à la Chine », prévient Emmanuel Macron. On pense alors aux seuls domaines du luxe et de l’automobile...Mais la France et les autres pays occidentaux y gagnent pour leurs importations devenues moins chères. Alors la France n'a rien à craindre du géant chinois.

Le poids de la Chine est à relativiser pour les économies occidentales, comme le rappellent les économistes de Natixis :

Les raisons sont multiples :

- « On mesure le poids des économies dans le PIB mondial en parité de pouvoir d’achat (PPA) et, selon ce critère, la Chine est la première économie du monde.

Mais ce classement est un artifice. Il consiste à rapporter le PIB d’un pays au pouvoir d’achat intérieur. Comme le PIB contient des exportations, beaucoup pour un pays comme la Chine, et que le niveau de vie domestique dans les émergents est plus faible que dans la plupart des économies cibles de leurs exportations, le PIB PPA surestime le revenu produit par les émergents.

Mesurées en dollars courants, les richesses produites par les États-Unis (17 000 milliards de dollars) restent ainsi plus grandes que celles produites par la Chine (10 400 milliards de dollars).

Mesurée en PIB par habitant, la Chine reste donc derrière les pays développés. »

-La France est peu dépendante de la Chine (6,5 % de ses exportations), l’Allemagne guère plus, à hauteur de 11%.

-Et surtout, nombreux sont les analystes à s'accorder sur le fait que la bourse de Shanghai ne reflète guère l'économie réelle de ce pays! En d'autre termes, de façon voilée, à nous signifier que les médias occidentaux font de la désinformation pour nous faire oublier notre sinistre situation économique...

En réalité, l’impact de la tourmente financière et du ralentissement de l’économie chinoise pour la France et les économies européennes ne devrait pas aller « au-delà de quelques dixièmes de points » de PIB, selon le ministre français de l'économie Macron.

Certes, de l'avis général, les assouplissements monétaires de la PBOC ne suffiront pas à l'activité économique, les investissements et la consommation.On peut s'attendre à de fortes interventions supplémentaires du gouvernement chinois, notamment en termes de relance budgétaire et de dépenses publiques dans le prochains mois selon Bloomberg News.

Le «  krach » du « lundi noir » ne sonne donc pas le glas de l’économie chinoise non plus.

Si l'on peut craindre pour l'activité mondiale de la Chine ,

rassurons-nous. La Chine compte pour plus de 13% de l'activité mondiale et reste à près de 8% de croissance. N'attaquons donc pas systématiquement le géant chinois à tort !






Après des décennies de croissance à deux chiffres, les analystes occidentaux doivent savoir que l'avenir de la Chine se fera par une hausse de l'activité plus modérée mais plus durable... soutenue par la consommation intérieure, plutôt que par les exportations.

La faute à qui ?

Essentiellement aux pays développés qui sont en grande récession , Europe et États-Unis en particulier, qui restreignent leurs importations devenues pourtant moins chères.

Et à quand une France à 8% de croissance alors qu'elle peine déjà à rester à 0% avant le grand plongeon dans l’abîme sans fond?!
